More than 450 runners will compete across the marathon, half-marathon, 10-kilometre and five-kilometre events between Town Beach and North Brother mountain.
Event director Mick Maher said numbers were similar in 2021 to what they were in 2019 when the festival was last held.
"It's been a difficult one because so many people have been reluctant to book into an event early because of how many events have been cancelled," he said.
"We're fortunate that with the COVID guidelines from November 8 stating up to 1000 people can participate, we're fully covered."
The marathon and team relay will start at 6am from Town Beach in Port Macquarie before the half-marathon starts on the sand (Bundella Avenue) on the southern side of the Lake Cathie bridge at 7.30am.
The 10-kilometre runners will start at North Haven Surf Life Saving Club at 8.30am before the five-kilometre runners start their trek up the mountain from Laurieton Reserve at 9.30am.
"We're expecting everyone to be finished by 1pm, with the first half-marathon and marathon runners to finish around 9.30 and 10am," Mr Maher said.
"Fortunately even with the issues with COVID in the community at the moment, because of our high vaccine rates it's still pretty safe."
