The woman was hit on William Street at about 12.55pm.
Two ambulance crews attended the scene and treated the woman for a laceration to her head and hip pain before taking her to Port Macquarie Base Hospital for further assessment. She was in a serious but stable condition, a NSW Ambulance spokesperson said.
It is the third pedestrian to be hit by a vehicle in Port Macquarie in the last month.
Last week, a teenager was treated for serious injuries after being struck by a car on the corner of Park and Buller Streets.
He was treated for chest, shoulder and leg injuries.
