The youth, who was believed to be visiting Port Macquarie, hired a surfboard at the beach on Anzac Day, April 25.
He entered the water but was later found unconscious by beachgoers.
The youth was treated by volunteer surf life savers before paramedics arrived at the scene. The injured surfer was then transported by ambulance to Port Macquarie Base Hospital.
Port Macquarie Surf School surf coach Wayne Hudson said the youth did not appear to be an experienced surfer.
"He hired a board around 1.15pm," Mr Hudson said
"I saw him in the water, about waist deep, but he must have caught a wave.
"He may have wiped out, hit his head on the board and been knocked out.
"First aid was performed by surf rescue volunteers and an ambulance was called."
Mr Hudson said he'd not seen this type of incident at Flynns Beach before.
"The surf was about two to three foot, it must have hit him in the wrong spot," he said.
"I've never seen something like this happen before with someone being knocked out. It was very quick.
"He did seem to be getting more conscious as time went on. Everyone hopes he is okay."
