Catholic Care of the Aged (CCA) and Centacare will change to St Agnes' Care & Lifestyle, effective from Wednesday, September 16.
St Agnes' Catholic Parish CEO Adam Spencer said the name change has been a culmination of joint organisational and operational activities between the two service areas of the Parish.
"Both CCA and Centacare have delivered great services to the community for decades and over the past few years have benefited from working more closely together and learning from each other," he said.
"The new name St Agnes' Care & Lifestyle better reflects what the two entities offer collectively, which is great care for those that need it with the lifestyle they want."
CCA has provided services in residential aged care, retirement living, home care and respite care, while Centacare provided offerings across NDIS, community housing, therapy services youth and family support programs.

"Bringing these services together under one name means we have a truly unique service offering that cannot be replicated," Mr Spencer said.
St Agnes' Care & Lifestyle general manager Bronwyn Chalker said the new name builds on the legacy of CCA and Centacare.
"Both CCA and Centacare have a great track record for the delivery of service to those in the community who need it most," Ms Chalker said.
"It is the way that we deliver our services, known as our service model, which builds trust in the community reinforcing our reputation as a high quality care provider. Our service model focuses on the four principles of empowerment, choices, connections and spirituality as the basis for all that we do," she said.
Ms Chalker said the while it was preferred to launch the new name with staff, clients and their families in person, it was not possible.
"COVID-19 prevented us from organising an event where we would bring everyone together to celebrate the launch of our new name."
"Instead we have invited everyone to watch a video on our new website where viewers get an understanding of the depth of services St Agnes' Care & Lifestyle offers with some engaging stories from staff and a client," Ms Chalker said.
