They scored the first and last tries of the match, but Forster's Ash Alcorn proved the difference with two tries of her own in the two-point victory.
Jess Jobson also found the line as the Hawks became the first team into the grand final while the Breakers now face Wauchope next Sunday.
Breakers coach Troy Roach admitted his team were "out-enthused", but would learn plenty from their first loss of the year.
"We learned we need to be a bit more enthusiastic and it's hard for these girls with the amount they've won over the years to turn up every week and continue to win," he said.
"Eventually someone's going to be more enthusiastic than you and eventually they're going to play better football than you.
"I'd prefer to lose this one than have that happen in two weeks' time and hopefully we meet them again."
Roach said Port City had their chances, but struggled in the blustery conditions.
"We couldn't attack as expansively as we usually do, but it was the same for Forster as well, they just handled the conditions a bit better than we did," he said.
"That undefeated run ... the monkey's off the back."
When Larissa Ward scored with four minutes remaining to close the margin to two points, the Breakers had their chances to win the match.
They also pressed the Forster line in the dying seconds before a Jazzy Wilbow bomb bounced over the dead-ball line.
"We had our chances; we prepared pretty good all week but were beaten by a better team on the day," Roach said.
