The service is at St Thomas' Anglican Church Port Macquarie.
A non-denominational event, the service will celebrate the lives of the many wonderful people who were assisted by the Palliative Care Unit during their period of illness.
It will also provide an opportunity for families, friends and staff to remember the special people they have lost.
Mid North Coast Local Health District chaplain Leanne Smith says that families really appreciate the remembrance services held by the health district.
"We find that staff and families really like having an opportunity to reconnect," Ms Smith said.
"The service is a really special way for families to remember their loved one.
"Palliative care is about improving the quality of life for patients, their families and carers by providing care that addresses the many needs of people with a chronic, life limiting or terminal illness.
"These needs include physical, emotional, social, cultural and spiritual needs."
Afternoon tea will be provided after the service.
For catering purposes, those wishing to attend the service are asked to RSVP to the Palliative Care Unit on 6580 8040 or to Chaplain Leanne Smith 0467 796 395.
