The incident occurred at the Bago Road and Pacific Highway intersection at about 11pm.
Police said the leaner driver of the Toyota Corolla turned into the path of the southbound truck from Bago Road.
A 17-year-old male from Taree and his 77-year-old grandmother were both taken to Port Macquarie Base Hospital with minor injuries both in a stable condition.
One lane of the Pacific Hwy was closed to traffic until 1am as crews worked to clean up.
