Just over 20 obstacles along a five kilometre coastal course tested the endurance of almost 500 participants.
They encountered a range of challenges including a climbing wall, crawling under a net, weaving between poles in the waves, slime, flour bombs and immersing themselves in icy water.
The event is expected to raise about $15,000 with the proceeds shared between Marine Rescue Port Macquarie and headspace.
Tacking Point Lions Club and Aurora Lions Club put on the endurance event in conjunction with Marine Rescue Port Macquarie and headspace.
Port Macquarie Sand Mudder Committee chairman Rob Bruce said the sand mudder was sensational.
"It is not a tough mudder but it is still hard," he said.
Mr Bruce said sand mudder was not a race but rather an endurance event against participants and the course, how many obstacles they could complete and how much fun they could have along the way.
He said the annual event would grow into the future with the aim of extending the participant reach beyond Port Macquarie.
"The event is run by people from Port Macquarie and all the money stays in the charities of Port Macquarie," Mr Bruce said.
